Honestly, for a mod it’s quite good - it’s no replacement for actual NPCs when they’re added but it’s decent enough to play about with. Fair warning - It’s clunky and sometimes very buggy, although my experience has been much better than others. I find keeping spawns to a minimum helps. And the best settings I found for me so far are, suvivors spawn rate at the lowest. 5% or 0% chance of spawning with guns (because there is always a guy with a shotgun making noise at 2 in the morning attracting zeds). For the rest of the settings I don't have a specific preference, maybe increase melee spawn.
